Titan quests (e.g. Defend North Kryta Province) or Titan mission (e.g. Hell's Precipice)?
The primary failure reasons don't change much either way. In order, the greatest reasons that people fail:
1) Multiple Sparks of the Titan roast them alive with overwhelming fire damage. Rodgort's Invocation, given the recent augmentation, is a huge problem.
2) They break down greater titans into large and unmanagable groups of lesser titans. In many cases, a lesser titan is far more lethal than the greater titan from which it spawned.
3) They antagonize larger groups of titans and run face-first into problems #1 and #2.
4) They don't disperse; a lot of the quest-related titans have vicious area of effect skills, and the greater mission-related titans cause burning to anyone nearby.
5) If they use hero monks for the missions, they don't consider disabling certain forms of condition removal. Hero monks are partial to using it immediately to solve burning... which comes back as long as the target is next to a greater titan.
6) They don't bring adequate healing period.
A ranger or a mesmer can help to prevent certain skills (e.g. Maelstrom, Chain Lightning, Rodgort's Invocation) from destroying your party. I've found that a single, highly proficient ranger can help to expand your survival more than any other profession. It's also worth considering if you should bring three monks.
